Health Care Directives

Do you have a health care directive in place should you become seriously ill with life support initiatives to be carried out on your behalf?

Have you named a power of attorney and or an alternate should you need someone to speak on your behalf?  Is this a family member that can be confided in should medical matters make it necessary?

Do you have Long Term Care Insurance or financing in place to take care of you should the need arise?


Living Arrangements

How and where do you want to live as you become older and the need for assistance becomes inevitable?

Is moving closer or “in” with family or friends an option?

Have you considered an assisted living or retirement facility that would be an acceptable living arrangement in lieu of your home?

Have you designated a care giver who could help you with daily tasks and or security at your current residence?


Final wishes and directives

Have you indicated what your wishes are at your passing?  Have you designated someone to see that your wishes are fulfilled?

Do you have a will or a trust that outlines these directives for the executor or the trustee?

Are the specifics for the funeral arrangement, burial or cremation complete?
